When was your Brachial Plexus Injury Clinic established? 1997. Identify what specialties related to brachial plexus injuries are represented by practitioners working in your clinic including physical/occupational therapists, if any. Two surgeons, 1 anesteziology and 1 therapist. What do you/your clinic specialize in treating? All kind of brachial plexus injuries, tumors, thoracic outlet send etc.. All kind of hand and upper extremity problems and orthopaedic microsurgey reconstructions ( vascularized bone grafts, nerve repairs and free flaps.) Indicate the total number of brachial plexus patients you/your clinic has evaluated since your establishment: Number of children (Obstetrical brachial plexus injuries): 320 Number of children (Trauma or other brachial plexus injuries): 28 Number of adults (Obstetrical brachial plexus injuries): 35 Number of adults (Trauma or other brachial plexus injuries): 212 State your philosophy regarding evaluation, referral and treatment of obstetrical brachial plexus injuries in children. In total palsy I do nerve surgery if the baby does not have biceps function after 3 months but in Erbs palsy I wait for the biceps function after 5 months. I generally do secondary operations after 2 years of age except early diagnosed shoulder subluxation or dislocation. State your philosophy regarding evaluation, referral and treatment of trauma or other brachial plexus injuries in children. In traumatic cases if there is root avulsion (clinical and MRI confirmed) I do surgery after 1 month or in the first year when the patient is seen in the clinic. After one year of trauma date I generally do secondary operations. State your philosophy regarding evaluation, referral and treatment of obstetrical brachial plexus injuries in adults. If the patient needs a secondary operation I do the operation if the patient wants it. They generally want the operation after sholl closure. I also do shoulder elbow and hand operations in one stage if possible. State your philosophy regarding evaluation, referral and treatment of trauma or other brachial plexus injuries in adults. If the patient has Horner sign + and MRI confirmed other root avusions I operate after 1 month but if the patient has Erbs palsy 1 wait biceps function 3 months. I do not do nerve repair after 6 months. State your philosophy regarding pain management and brachial plexus injuries. I use medication first if there is still pain I use medication pumps if there is still severe pain I can do posterior rhizotomy. Indicate research efforts (include dates please) conducted by you specific to brachial plexus injuries. We do not have research studies. Indicate any other facts that you feel would help families and individuals understand your practice. Families or the adult patient must know every detail of the surgery and the results. Do you accept any national/governmental payment for services or are you enrolled in any state medicaid plans? Please provide information on these plans and include which states you are enrolled in for medicaid services. I accept all kind of national or government payments. |
